Scope of Position:
• To assist individuals affected by the opioid epidemic in securing and maintaining occupations that will provide both
a living wage and a solid direction forward. Serving a 6 county region in NWPA.

Responsibilities

Essential Functions:
• Build a professional one-on-one working relationship with participants to help in eliminating challenges that are
hindrances to obtaining and retaining employment, including assessment, individualized attention, and
coordination
• Provide Career Coaching including the completion of the Individual Employment Plan (IEP), WIOA eligibility,
CASA’s, and Career Mapping to enrolled participants
• Accurately complete required documentation for WIOA enrollment
• Provide workshops for participants residing in rehabilitation centers
• Work directly in approved rehabilitation centers to provide services related to removing barriers to employment
• Refer customers directly to the PA CareerLink® for post rehabilitation services as part of their treatment plan
• Responsible for participation tracking and monitoring data entry into CWDS in accordance with activity codes and
barrier identification and resolution
• Update the CareerLink Referral Spreadsheet Report to provide status updates on new participants
• Assist in the preparation of weekly project reports (participation, placement, etc.)
• Create and maintain an environment that promotes opportunities for customers to hope for the future, accept
themselves, and provides tools for facing adversity
• Provide referred customers with information on recover friendly employers in the region
• Ensure recording keeping of all customer outreach and contacts through internal tracking systems
• Oversee participant activities and assist with workshop schedules
• Provide consistent and comprehensive case management and follow-up
• Provide individualized assessment of work, family and personal development needs and opportunities
• Provide referrals to appropriate community service or social service agencies when needed
• Maintain an organized database of clients and keep detailed documentation of all contacts with clients
• Coordinate participant post exit retention services and accommodate participant work schedules
• Document all participant contacts and activities in case notes and ensure that post-placement contacts are made
as required by specific contract
• Share information with appropriate staff and/or funding agencies
• Maintain participant files as specified by contractor
• Ensure participants follow through with planned activities as outlined in contract with funding agency
• Ensure that positive outcomes and goals are met as required by contract
• Other opportunities as available

Qualifications

Education and Experience Requirements:
• Ability to prioritize tasks and document visits and contacts
• Demonstrated telephone, computer, and written communication skills
• Ability to maintain flexibility as needs of contract or contractor require
• Social Work college degree or equivalent experience required
• Specialized Substance Abuse, Mental Health, or Trauma-Informed Care training and/or experience required
• A working knowledge of Microsoft Office
• Valid driver’s license, vehicle and clean driving record required
