  • To provide support and assistance to program participants in all areas related to gaining and/or retaining employment and career advancement
  • Due to the dynamics of working with individuals, this position necessitates an individual who is comfortable working in an environment that is constantly evolving
  • This position requires an organized individual who can efficiently handle several responsibilities simultaneously

Responsibilities

  • Responsible for participation tracking and monitoring data entry into CWDS in accordance with activity codes and barrier identification and resolution
  • Accurately complete required documentation for enrollment
  • Assist in the preparation of weekly project reports (participation, placement, etc.)
  • Work with government officials and partnering agencies to ensure appropriate documentation is completed and received
  • Create solutions to participant attendance problems
  • Maintain organized database of clients and keep detailed documentation of all contacts with clients
  • Coordinate participant retention services and accommodate participant work schedules
  • Document all participant contacts and activities in case notes and ensure that post-placement contacts are made as required by specific contract
  • Share information with appropriate staff and/or funding agencies
  • Maintain participant files as specified by contractor
  • Ensure participants follow through with planned activities as outlined in contract with funding agency
  • Help to ensure that positive outcomes and goals are met as required by contract
  • Conduct a comprehensive household assessment within 14 business days after the enrollment date
  • Assess participant’s social indicators of health (SIOH)
  • Create an Individual Employment Plan (IEP) in CWDS by utilizing a comprehensive household assessment
  • Ensure intensive case management through coaching, peer-to-peer experiences, and ongoing contact
  • Identify and refer the participant and/or their family to the appropriate local community resource(s) to achieve goals identified in the IEP
  • Advocate for the participant and their family when connecting to local community resources by providing participants referrals and follow-up
  • Provide access to at least one qualified Social Service Professional (SSP) to provide mental, social, physical, emotional, and behavioral health support
  • Record all referrals to the social service professional in CWDS along with outcomes.
  • Facilitate the transfer of participants to other DHS employment and training programs
  • Document excused absence time in the case narrative and devise make-up plans if necessary 
  • Identify, provide access to, and document the use of supportive services
  • Maintain case notes following DHS policies in the system of record
  • Advocate for participation at DST meetings
  • Verify employment hours through paystubs, documentation from the employer, or DHS-approved Employment Verification Form (EVF)
  • Identify participants whose TANF cash budget has closed and place them in the Job Retention phase of the program. 
  • Verify employment at 6 months (183 days) and 12 months (365 days) after retention begins

Qualifications

  • Ability to prioritize tasks and document visits and contacts
  • Demonstrated telephone, computer and written communication skills
  • Ability to maintain flexibility as needs of contract or contractor require
  • College degree or equivalent experience required
  • Customer service experience preferred
  • A working knowledge of MS Office
